THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This notice describes the privacy practices that apply to health information handled through the Metabolic Regen MD platform, including by the independent provider groups that deliver your care and by Metabolic Regen LLC as their business associate.
How we may use and disclose your health information
- Treatment: for example, sharing your intake with your provider and your prescription with the pharmacy.
- Payment: to bill and collect for services.
- Healthcare operations: quality review, training, and compliance.
- As required by law, for public health and safety, health oversight, legal proceedings, and law enforcement as permitted.
Other uses, including marketing and the sale of PHI, require your written authorization, which you may revoke.
Your rights
- Get a copy of your health record
- Ask us to correct your record
- Request confidential communications
- Ask us to limit what we use or share
- Get a list of those with whom we’ve shared information
- Get a copy of this notice
- Choose someone to act for you
- File a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Our responsibilities
We are required by law to maintain the privacy and security of your PHI, to notify you promptly of a breach, to follow this notice, and not to use or share your information other than as described here unless you authorize it in writing.
