Telehealth Consent

What telehealth is

Telehealth means the delivery of healthcare services using electronic communications, including secure questionnaires, messaging, and video, between you and a licensed provider who is not in the same location.

Who provides your care

Your care is provided by an independent clinician licensed in the state where you are located, through our partner provider networks (CuraDocs, The Network Health, and Zira Medical). Metabolic Regen MD does not provide medical care.

Benefits and limits

Telehealth can improve convenience and access. It has limits: your provider cannot perform a physical examination, and information may be incomplete. Your provider may require a video visit, lab tests, or an in-person evaluation, or may decline to treat you through telehealth.

Risks

Risks include delays from technical problems, incomplete information leading to errors in judgment, and, rarely, security breaches despite safeguards.

Your responsibilities

You agree to provide accurate and complete information, to be physically located in a state we serve during your visit, to follow your provider’s instructions, and to seek in-person or emergency care when needed.

Your rights

You may decline or withdraw from telehealth at any time without affecting your right to future care. You may request your medical records.
