The purpose of this Informed Consent to Telehealth is to inform you about the nature of telehealth services provided through the Metabolic Regen platform and to obtain your consent to receive care in this manner. Telehealth services are delivered by independent, licensed healthcare providers of affiliated professional practices (the “Provider Groups”), which are managed by DrTelx LLC and operate on the Rimo Health telehealth platform. Metabolic Regen MD, LLC does not itself provide medical services and does not employ the treating providers.
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a provider and a patient who are not in the same physical location. Telehealth may include, but is not limited to, the review of your medical history and intake information, secure messaging, audio and video communication, the electronic transmission of medical information, and the issuance of prescriptions when clinically appropriate.
Your provider will use the information you supply to evaluate your condition and determine whether treatment through telehealth is appropriate for you.
The anticipated benefits of telehealth include improved access to care from your home or another convenient location, more flexible scheduling, reduced travel and wait times, and the ability to receive follow-up care and support without an in-person visit. Telehealth can make it easier to obtain timely evaluation and ongoing management of your care.
As with any medical service, there are potential risks associated with the use of telehealth. These include, but are not limited to: information transmitted may not be sufficient to allow for appropriate medical decision-making; a provider may not be able to conduct a physical examination that could otherwise reveal relevant information; delays in evaluation or treatment could occur due to technology failures or interruptions; in rare cases, security protocols could fail, causing a breach of privacy; and a provider may determine that your condition is not appropriate for telehealth and that in-person care is required.
You understand that these limitations exist and that your provider will advise you if telehealth is not appropriate for your situation.
Telehealth is not appropriate for medical emergencies. If you are experiencing a life-threatening emergency, call 911 immediately. If you are experiencing a mental health crisis or thoughts of self-harm, call or text 988 to reach the Suicide and Crisis Lifeline.
You are responsible for following up with your provider and for seeking in-person or emergency care when needed. Telehealth does not replace your relationship with a primary care provider or local emergency services.
Reasonable administrative, technical, and physical safeguards are used to protect the security and confidentiality of your personal and health information transmitted through the platform. Your information may be shared with providers, pharmacies, and service providers as necessary to deliver your care, and as otherwise permitted or required by law. Please review our Privacy Policy for details on how your information is collected, used, and protected.
By consenting, you acknowledge and agree that: you have the right to withhold or withdraw consent to telehealth at any time without affecting your right to future care; the laws that protect the privacy and confidentiality of medical information also apply to telehealth; you understand that no specific outcome or result is guaranteed; you are responsible for providing complete and accurate information about your health history, medications, and symptoms; and you understand that a provider may determine that you are not a candidate for treatment through the platform.
You may withhold or withdraw your consent to receive telehealth services at any time by contacting us, without affecting any care you have already received. Withdrawing consent may, however, limit our ability to provide certain services to you.
