You have the right to

• Be treated with respect, dignity, and without discrimination
• Know the name and licensure of the provider who treats you
• Receive clear information about your diagnosis, treatment options, risks, benefits, and costs before you agree to treatment
• Refuse or stop treatment at any time
• Privacy and confidentiality of your health information
• Access and receive copies of your medical records
• Know the pharmacy that fills your prescription and choose to have a prescription sent to a different pharmacy where permitted
• Voice complaints without retaliation

You are responsible for

• Providing complete and accurate health information
• Following your treatment plan or telling your provider if you do not
• Reporting side effects or changes in your health
• Seeking emergency care when needed
